1. Identity of the Data Controller
Pursuant to the Law on the Protection of Personal Data No. 6698 (“KVKK”), DentSava Oral and Dental Health Clinic, in its capacity as the data controller, may process your personal data for the purposes described below.
2. Your Personal Data Processed and the Purposes of Processing
The identification data (First Name, Last Name), contact information (Phone, Email), and health data you provide to us;
- Scheduling, confirming, and sending reminders for appointments,
- To provide medical diagnosis, treatment, and care services,
- To share requested information with the Ministry of Health and other public institutions and organizations in accordance with applicable legislation,
- To manage patient relations and carry out billing processes
are processed strictly for these purposes.
3. Method of Collection and Legal Basis for Personal Data
Your personal data is collected electronically via the form you fill out on our website. This data is processed in accordance with Articles 5 and 6 of the Personal Data Protection Law (KVKK);
"Directly related to the conclusion or performance of a contract," "Necessary for the data controller to fulfill its legal obligations," and, for your health data, "Your explicit consent."
4. Transfer of Personal Data
Your personal data may be transferred, within the conditions specified in Articles 8 and 9 of the KVKK, to authorized public institutions and organizations (the Ministry of Health, the Social Security Institution [SGK], judicial authorities), private legal entities authorized by law, and our service providers.
5. Rights of the Data Subject (Article 11 of the KVKK)
As the data subject, you have the right to: learn whether your data is being processed; if so, request information regarding such processing; learn the purpose of the processing and whether it is being used in accordance with that purpose; know the third parties to whom your data has been transferred within or outside the country; request the correction of any incomplete or incorrect data; and to request the erasure or destruction of your personal data under the conditions set forth in Article 7 of the KVKK.
